Developmental Disability Services (DD Waiver)
Brightstar Homes & Services, Inc. offers services for individual with developmental disabilities and their families within Metro-Atlanta area and surrounding counties. Brightstar Homes & Services, Inc. provides services to the DBHDD population through the Medicaid Waiver Program (NOW & COMP).
Brightstar Homes & Services, Inc. provides Community Residential Alternatives services (CRA) to individuals in the comforts of a structured home setting environment, with 24-hour supervision. Our CRA HOMES and HOST HOMES (Life Sharing) are designed to provide the comforts of a home setting with a Direct Support Care Provider and one to three roommates (CRA HOME) and one to two individuals in (HOST HOME). Brightstar Homes & Services, Inc. strategically addresses the needs and desires of individuals served by providing initial nursing assessments and bi-monthly nursing visits by Registered Nurse or LPN, monthly DDP Professional home visits and bi-weekly visits by the Program Managers to ensure that all needs, hopes and desires are being addressed by the agency for each and every individual served.
Brightstar Homes & Services, Inc. is committed to working diligently with individuals and families to identify strength, needs, and work towards progress to achieving individual’s hopes, dreams and desires. We help Individuals to become integrated into the community by motivating and allowing them to become more involved in the activities of their choice.
Behavior Support Management (BSM)
Brightstar Homes & Services, Inc. Provides Behavior Support Management which includes the development of a Behavior Support Plan, Safety Plan, and or Crisis Plan by a qualified Behavior Specialist in the event there is crisis involving a individual. These plans are implemented to take all measures to keep the individual safe. Behavior specialist ensures that staff are trained upon hire, when changes are made to either plan and annually.
Community Access Services
Brightstar Homes & Services, Inc. Provides Community Access Services which are designed to help participants acquire, retain or improve self-help, socialization and adaptive skills. Services for those individuals that would like, or need, more independence services. These services are designed during and ISP (Individual Service Plan) to meet the individual’s independent needs. These services can include a wide variety of community-based activities, with one on one staff (CAI) and services that are provided to groups of individuals (CAG). These services allow individuals with disabilities more access to participate in the activities of their choice, in their own community with the ability to live a more independent and productive life. Our Day Service Program is offered from 8:30am- 2:30pm, Monday-Friday.
Developmental Waiver Services (DD)
Brightstar Homes & Services, Inc. offers services for individuals with developmental disabilities. The individual’s services are based on the service through the New Options Waiver Program (NOW) and Comprehensive Supports Waiver Program (COMP) which is a Medicaid waiver that provides individual with developmental disabilities the opportunity to receive services in home setting and in the community.
Waiver Services Include:
- Community Residential alternative (CRA) up to three individuals
- Host Homes (HH)-Life Sharing only two individuals
- Individualized Community Access (CAI)
- Community Access Group (CAG)
- Behavior Support Management (BSM)
New Options Waiver Program (NOW) Covered services are:
- Individualized Community Access (CAI)
- Community Access Group (CAG)
- Behavior Support Management (BSM).
